Completing this form will help to ensure the following:

• Confirm jersey numbers for returning players
• Capture preferred jersey numbers from new players
• Reduce the chance for duplicate numbers on the same team
• Speed up the roster registration for NCAA Sanctioned tournaments
• Confirm your shooting shirt for Winter Conditioning participants (Must have run both Saturdays for the free shooting shirt)
